Metric · Story · Coaching

Culture Health Index.

Not a satisfaction survey. Rate each dimension 1–5, then — more importantly — name the story behind the number and the coaching action it calls for. A low number is a starting point, not a verdict.

Work through one dimension at a time. Ask different people what they have experienced, identify the evidence behind the rating, and agree on a first action. This conversation tool does not calculate an overall score.
How to use the six areas

Read experience, not just a score.

These areas are prompts for a team conversation. Ask several people what they have actually experienced, compare perspectives, and name the conditions leaders can change. A high rating in one area does not cancel a concern in another.

01 · Mission & MetricsDo people know why the work matters and what the measures reveal or miss?
02 · CommunicationDoes information travel in time, and can concerns travel back up?
03 · CollaborationCan people work across boundaries and complete handoffs together?
04 · Coaching & FeedbackDo people receive specific support and chances to learn from the work?
05 · Accountability & EmpowermentAre expectations matched by authority, resources, and follow-through?
06 · Proactivity & AdaptabilityCan the team notice change early, adjust, and learn without blame?

What do the numbers mean?

1 Repeated gaps or harm · 2 Gaps outweigh strengths · 3 Mixed or uneven experiences · 4 Usually supportive, with exceptions · 5 Consistently supportive across people and situations

Choose the number that best fits the evidence you have. If people experience the area differently, record that difference in the story rather than averaging it away.
